Inclusion criteria

Inclusion criteria: Patients of both genders, age from 18 years; Signing of the informed consent form (TCLE); Hospitalized patients with medical diagnosis of decompensated HF independent of etiology.

Exclusion criteria

Exclusion criteria: Patients with cognitive neurological impairment and / or who are already participating in other studies with educational interventions will be considered.

Design outcomes

Primary

MeasureTime frame
A score above 70 points (considered adequate self-care) of the maintenance, management and confidence measures of self-care in heart failure at discharge, 30 days and 90 days after discharge. These measures will be carried out by applying the validated questionnaire in Brazil "Self-care of Heart Failure Index, version 6.2". The questionnaire has 22 questions divided into three scales: Self-care maintenance (10 items), Self-care management (6 items) and Self-care trust (6 items). Responses to items range from "never / rarely" to "always / daily" to Self-Care Maintenance Scale; "Unlikely" to "very likely" scale of self-care management and "not confident" to "extremely confident" to scale Trust in self-care. The topics addressed in the questions are: weight and edema measurement; physical activity; Assiduity in the consultations; diet; medicines; Recognition of signs and symptoms of decompensation; Management activities of signs and symptoms and confidence for decision making. The scores for each subscale will be calculated separately, each one ranging from 0 to 100 points, with a score of 70 or more points considered as adequate self-care. There will be a comparison of the means of the self-care scores between the intervention and control groups by the T-Student test.

Quality of life score below 52.5 (considered as poorly affected quality of life) at the time of discharge, 30 days and 90 days after discharge. This will be done by applying the validated questionnaire in Brazil "Minnesota Living With Heart Failure". The questionnaire consists of 21 questions related to limitations presented by patients with heart failure. The scale of responses for each question ranges from 0 to 5, where 0 (zero) is without limitation and 5 (five) maximum limitation. The questions are divided into physical dimensions (from 1 to 7, 12 and 13), emotional dimension (from 17 to 21) and socio-affective issues (from numbers 8, 9, 10, 11, 14, 15 and 16) Added to the previous dimensions, generate the total score (105 points). There will be a comparison of the means of quality of life scores between the intervention and control groups by the Student-T test.
