This unicenter, open-label, randomized, controlled pilot study will include 25 patients in mechanical insufflation-exsufflation (MI-E) group and 25 patients in control group. The randomization will be

Age
18 Years to No maximum

Inclusion criteria

Inclusion criteria: Pacientes aged more than 18 years; mechanical ventilation for more than 21 days; mechanical ventilation through tracheostomy; fulfill criteria for weaning; respiratory stability; hemodynamic stability; uncontrolled infection; no severe mebolic disturb

Exclusion criteria

Exclusion criteria: Pneumothorax; subcutaneous emphysema; pneumomediastinum; thoracic trauma; haemoptysis; intracranial hypertension; readmission to ICU after discharge already tracheostomised; palliative care; refusal to sign the informed consent by the responsible family member

MeasureTime frame
To evaluate weaning success rate, defined as withdrawal from mechanical ventilation, characterised by the presence of spontaneous ventilation through the tracheostomy, without connection to the ventilator, during the 24 hours of 5 consecutive days
