Inclusion criteria
Inclusion criteria: Pacientes aged more than 18 years; mechanical ventilation for more than 21 days; mechanical ventilation through tracheostomy; fulfill criteria for weaning; respiratory stability; hemodynamic stability; uncontrolled infection; no severe mebolic disturb
Exclusion criteria
Exclusion criteria: Pneumothorax; subcutaneous emphysema; pneumomediastinum; thoracic trauma; haemoptysis; intracranial hypertension; readmission to ICU after discharge already tracheostomised; palliative care; refusal to sign the informed consent by the responsible family member
