Inclusion criteria
Inclusion criteria: For the first objective Source documents of maternal and neonatal health services related to antenatal care, delivery services, postnatal care, family planning services and immunization records will be included. All records generated during the three-month baseline period and the three-month intervention for end line period will be eligible for selection. For the second objective Aggregated monthly HMIS reports sent from health posts to health centers and from health centers to district health offices during the study period will be included. For the third objective All healthcare workers in intervention and control facilities who have a role in maternal and neonatal health data documentation and supervision, who present during baseline and end-line data collection and give consent for participation will be included. For the fourth objective Facility leaders, department heads, trainers, HMIS focal persons and healthcare workers who have direct experience with the intervention or oversight of data quality will be included.
Exclusion criteria
Exclusion criteria: For the first objective Records from service units that are not included in HMIS reporting, records with incomplete patient identifiers that prevent linking patient charts to register entries, and records from periods of major facility disruption will be excluded. Client records with missing folders will also be excluded For the second objective Non-routine performance reports such as financial reports and reports from months outside the study period will be excluded For the third objective Healthcare Workers on extended leave during data collection and those who decline to provide the consent will be excluded For the fourth objective Individuals who had no knowledge of or involvement with the intervention will be excluded
