Inclusion criteria
Inclusion criteria: Own a smartphone Able to use WhatsApp for basic functions (receive and view short video clips, send voice notes and text messages). Proficiency will be confirmed during a 10-minute orientation session prior to enrolment
Exclusion criteria
Exclusion criteria: Participants will be excluded if they have: 1.Any "red flags" indicating serious spinal pathology (e.g., tumour, fracture, cauda equina syndrome). 2.A history of spinal surgery in the past year. 3.Cognitive impairments that would prevent them from following instructions. 4.Concurrent participation in other formal treatment programmes (e.g., chiropractic, acupuncture) for the same condition. Pregnancy.

| 1. Pain Intensity: Numeric Pain Rating Scale (NPRS); Participants will rate their average pain over the last week on a scale of 0–10. The study defines the MCID for the NPRS as 2.0 points (cite source). 2. Functional Disability: Oswestry Disability Index (ODI) version X the ODI is a spinespecific, validated instrument widely used in low back pain research and sensitive to clinically meaningful change. The ODI total score (0–100%) will be calculated and used as the primary disability outcome; cite validation and responsiveness references and the ODI MCID (insert citation and numeric MCID). Language validation or translation procedures (forwardbackward translation and pilot cognitive testing) will be performed where no validated Twi version exists. 3. Health-Related Quality of Life: EQ5D5L index scores and EQ5D utility values will be calculated using the Ghana value set if available; if not available, a clearly justified substitute value set will be used and referenced. | — |

| Direct costs transport fares to clinic, outofpocket physiotherapy session fees, medication and consumable costs, cost of data bundles for remote participation, cost of materials or equipment purchased for exercises, and caregiver outofpocket expenses.
