Inclusion criteria

Inclusion criteria: • Age = 18 years • Diagnosis of ARDS according to Berlin definition • PaO2/FiO2 = 300 mmHg with PEEP = 5 cmH2O • Invasive mechanical ventilation in pressure controlled mode • Enrollment within 24 hours of ARDS diagnosis ARDS diagnosis will be confirmed independently by two ICU physicians based on clinical and radiological criteria.

Exclusion criteria

Exclusion criteria: • Pregnancy • Chronic home mechanical ventilation • DNR/DNI status at enrollment • Enrollment in interventional ventilator trials • ECMO initiation within the first 72 hours Early deaths and patients ventilated<3 days will NOT be excluded to avoid survivorship bias.
