Inclusion criteria

Inclusion criteria: Older adults aged 60 years and above • Presence of one or more modifiable stroke risk factors • Ability to communicate verbally in Arabic • Ability to participate in educational sessions and follow-up assessments • Agreement to participate and provide informed consent

Exclusion criteria

Exclusion criteria: • Severe cognitive impairment or diagnosed dementia • Severe hearing or visual impairment preventing participation • Acute medical instability requiring immediate hospitalization • History of severe psychiatric illness interfering with participation • Participation in another behavioral intervention study during the study period

Outcome Name: Revised Framingham Stroke Risk Profile (R-FSRP) Description: Change in Revised Framingham Stroke Risk Profile (R-FSRP) score from baseline to 16 weeks post-intervention to evaluate reduction in estimated 10-year stroke risk among older adults receiving the nurse-led 4Ms-based educational intervention.
