Inclusion criteria
Inclusion criteria: • Adults =18 years having an indwelling arterial line. • Intubated and mechanically ventilated =12 hours • Clinically stable: o FiO2 = 0.6 o PEEP = 10 cmH2O o No spontaneous effort • Expected ongoing ventilation for =4 hours
Exclusion criteria
Exclusion criteria: • PaO2/FiO2 < 100 mmHg • Hemodynamic instability requiring escalating vasopressors • Pneumothorax, bronchopleural fistula, or air-leak syndrome • Raised intracranial pressure, severe arrhythmias, or inability to tolerate suctioning • Pregnancy • If the arterial line is removed for clinical reasons before completing all study periods, the participant will be withdrawn from further interventions, but data collected up to that point will remain included in the intention-to-treat analysis.
