Measurement of IWI and MVRT during SBT

Inclusion criteria

Inclusion criteria: 1. Adult patients (=18 years old) on mechanical ventilation for = 48 hour. 2. Clinically ready for SBT according to standard weaning criteria:- A. Underlying Condition Stable: The illness that led to respiratory failure is improving or resolved. B. Adequate Oxygenation: Low FiO2 (<0.5) and PEEP (5-8 cm H2O). C. Hemodynamic Stability: Stable blood pressure, heart rate, and no significant arrhythmias. D. Mental Status: Alert and able to protect airway (MOVE criteria: Mental status, Oxygenation, Ventilation and Expectorate). E. Minimal Secretions. 3. Adequate cough reflex. 4. Patients are to be synchronized with the MV before the SBT.

Exclusion criteria

Exclusion criteria: 1. Pre-existing neuromuscular diseases affecting respiratory function (myasthenia gravis, spinal cord injuries and amyotrophic lateral sclerosis). 2. Uncontrolled hemodynamic instability or ongoing shock requiring high-dose vasopressors. 3. Severe cognitive impairment or brain injury preventing participation in the study. 4. Tracheostomy placed within the last 48 hours or expected need for long-term tracheostomy. 5. Active infection with sepsis or multi-organ failure associated with uncontrolled shock or ongoing hemodynamic instability requiring high-dose vasopressors. The study population represents hemodynamically stabilized patients deemed clinically appropriate for SBT; therefore, findings apply to the weaning-assessment phase rather than early unstable critical illness.
