Inclusion criteria
Inclusion criteria: • Age =18 years old • Have been invasively mechanically ventilated via an endotracheal tube for =12 hours • Require a PEEP =10 cmH2O at the time of screening • Are hemodynamically stable with continuous arterial pressure monitoring • Have an indwelling arterial catheter
Exclusion criteria
Exclusion criteria: Hemodynamic instability requiring escalating vasopressors Severe hypoxemia (e.g., PaO2/FiO2 <100 mmHg or SpO2 <90% despite FiO2 =0.6) Pneumothorax, bronchopleural fistula, or active air leak Raised intracranial pressure (contraindication to suctioning) Severe arrhythmias Inability to tolerate suctioning (e.g., severe bronchospasm) Pregnancy Lack of an indwelling arterial line

| Gas Exchange: PaO2/FiO2 ratio and SpO2 at all timepoints; SpO2 nadir. Arterial blood samples will be analysed at Baseline (mmediately before suctioning), 0 minutes (Immediately after the suction procedure (within 60 seconds), 5, 15, 30, and 60 minutes post-suction.;Respiratory Mechanics: Dynamic compliance and driving pressure.;Safety: Incidence of hypoxemia (SpO230s or 20% drop from baseline), and arrhythmias. ;Recovery: Time for LUS score within 2 points of baseline and PaO2/FiO2 to return to within 5% of baseline. | — |
