Inclusion criteria
Inclusion criteria: - Age 18 years or older - Outpatient at a participating study site - A diagnostic test has been ordered by a participating clinician as part of routine care - Able to provide informed consent in English or an approved local-language version - Willing to have the diagnostic workflow processed through Pisgah - Has access to a phone or agrees to assisted access through approved site procedures
Exclusion criteria
Exclusion criteria: - Age under 18 years - Emergency, critical, or life-threatening condition where study procedures could delay care - Inpatient admission at the time of enrollment - Unable to provide informed consent and no approved representative consent process applies - Prisoner or other institutionalized person under coercive control - Clinician judges that participation may interfere with care
