Inclusion criteria
Inclusion criteria: Adults aged 18 years and above residing in the selected study communities. Individuals with hypertension, defined as a clinical history of hypertension, current use of antihypertensive medication, or measured blood pressure =140/90 mmHg. Individuals with pre-hypertension, defined as systolic blood pressure 120–139 mmHg and/or diastolic blood pressure 80–89 mmHg according to the Joint National Committee VII classification. Individuals with normal blood pressure (neither hypertensive nor pre-hypertensive), allowing evaluation of prevention as well as control outcomes. Individuals capable of providing informed consent and willing to participate in the study assessments. Residents of the selected communities who are available for baseline, post-intervention, and follow-up assessments.
Exclusion criteria
Exclusion criteria: Individuals with a history of stroke. Individuals with terminal illnesses, such as advanced cancer or congestive heart failure. Individuals unable to provide informed consent, including those with severe cognitive impairment. Individuals with severe sensory limitations that prevent participation in study procedures (e.g., severe visual or hearing impairment that would make participation in assessments impossible). Individuals unwilling to provide informed consent or participate in the study assessments.

| Hypertension knowledge and perception Assessed using the Hypertension Knowledge-Level Scale (HK-LS). Body Mass Index (BMI) Measured from height and weight to assess changes in body composition. Dietary behaviour Assessed using the Diet History Questionnaire (DHQ-III) to evaluate dietary patterns including salt and fat intake.
