Inclusion criteria
Inclusion criteria: 1. Adults =18 years. 2. Known diagnosis of hypertension, diabetes, or CVD for =12 months, 3. Receiving care at Mutare City clinics, 4. Resident in Mutare, 5. Able to provide informed consent. 6. Older adults (=65 years) are explicitly included. 7. Participants who indicate difficulty using digital tools may enroll with a nominated caregiver/tech support person willing to assist with study procedures and intervention use.
Exclusion criteria
Exclusion criteria: 1. Acute medical instability precluding participation at the time of enrollment; 2. Inability to provide informed consent after standard support; 3. No available caregiver when caregiver support is deemed necessary for safe participation.
