Inclusion criteria

Inclusion criteria: All adults with uncontrolled HTN who receive care at selected primary health facilities and can provide informed consent. Adults who have DM in addition to uncontrolled HTN will also be included.

Exclusion criteria

Exclusion criteria: We will exclude all mentally unfit clients with uncontrolled HTN who receive care at the respective health facility.

Design outcomes

Primary

MeasureTime frame
The change in systolic blood pressure (SBP) from baseline to 12-month follow-up.

Secondary

MeasureTime frame
The proportion of participants with controlled blood pressure (<140 mm Hg for systolic blood pressure and <90 mm Hg for diastolic blood pressure) at the last follow-up visit;Other outcomes (Change in HbA1c, reduced BMI, waist circumference, waist-hip ratio, blood pressure, presence of foot wounds)
