Inclusion criteria

Inclusion criteria: • Adult patients (= 19 years old). • Who have received invasive mechanical ventilation for more than 72 hours. • Who are clinically stable and deemed ready to undergo a Spontaneous Breathing Trial (SBT) and weaning attempt, according to the unit's protocol. • Written informed consent obtained from the patient or their legal authorized representative.

Exclusion criteria

Exclusion criteria: • Previous tracheostomy before the weaning attempt. • Severe neuromuscular disease affecting respiration (e.g., ALS, myasthenia gravis). • Death before extubation or within 48 hours post-extubation without reintubation. • Refusal or inability to provide consent.

The primary outcome of this study is to determine and compare the predictive accuracy of IWI and CORE index for reintubation within 48 hours of extubation (extubation failure)
