Inclusion criteria

Inclusion criteria: Adults aged =19 years Clinical diagnosis of acute cardiogenic pulmonary edema (ACPE) Radiologic evidence consistent with pulmonary edema (chest X-ray) Respiratory distress/dyspnea requiring non-invasive respiratory support SpO2 =92% on conventional oxygen therapy Hemodynamically stable (no immediate need for invasive ventilation)

Exclusion criteria

Exclusion criteria: Immediate need for endotracheal intubation Severe hemodynamic instability (e.g., shock/need for high-dose vasopressors) Altered mental status (GCS =12) or inability to protect airway Facial trauma/abnormalities preventing mask/cannula fitting Active upper gastrointestinal bleeding or high aspiration risk Contraindications to NIV/HFNC as per local practice Pregnancy
