Inclusion criteria

Inclusion criteria: • Radiopaque renal or proximal/mid ureteral stone =20 mm. • eGFR =60 mL/min/1.73 m². • Fit for ESWL. • Able to give informed consent and complete follow up.

Exclusion criteria

Exclusion criteria: • Pregnancy. • Active, untreated UTI. • Uncorrected bleeding disorder or anticoagulation that cannot be stopped. • Prior ipsilateral renal surgery. • Non radio-opaque stone without reliable imaging. • Inability to tolerate ESWL or required anesthesia.

Design outcomes

Primary

MeasureTime frame
•Stone free rate at 4 weeks: no residual fragment >4 mm on NCCT; residual fragments =4 mm considered clinically insignificant and counted as stone free.

Secondary

MeasureTime frame
Retreatment rate within 12 weeks, time to stone clearance, complication rate (Clavien–Dindo), pain scores (VAS), hospitalization rate, serious adverse events through 12 weeks, and quality of life (EQ-5D/SF-36).
