Inclusion criteria

Inclusion criteria: - 18 years and older - Diagnosis of stroke, confirmed by a physician, within the last six months - Modified Rankin Score between 2 and 4 - Able to identify at least 3 functional challenges - Provides consent

Exclusion criteria

Exclusion criteria: - Unable to regularly attend the Life After Stroke Centre (at least 2 x/week for 12 weeks)
