Inclusion criteria
Inclusion criteria: Stroke survivors aged 18 years and above Admitted in the neurological ward or clinics Primary diagnosis of ischemic stroke confirmed by CT/MRI Undergo usual discharge process Living with a family caregiver Has a mobile phone Can read and write Able to communicate National Institutes of Health Stroke Scale (NIHSS) 9 Modified Rankin Scale (mRS) 14, Expected to stay in the ward for 3-5 days, Expected to survive for 3 months
Exclusion criteria
Exclusion criteria: Survivors with previous stroke who were not admitted in neurology wards or visiting neurology clinics Survivors discharged against medical advice Survivors with end-stage organ failure Survivors with aphasia Survivors without a family caregiver Survivors who can’t read/write Survivors who have no mobile phone Survivors with malignant brain tumor
