Inclusion criteria

Inclusion criteria: Age = 18 years. Timing of emergency room presentation between 4.5 and 9.0 hours after stroke onset or at the time the patient awoke with stroke symptoms. Presence of large vessel occlusion involving either the internal carotid artery or the middle cerebral artery M1 segment. Pre-stroke Modified Rankin Scale (MRS) = 2. National Institutes of Health Stroke Scale (NIHSS) =6. General eligibility criteria for intravenous thrombolytic therapy other than time. MRI brain: DWI/FLAIR mismatch, which means the presence of an acute ischemic lesion visible on DWI and not visible on FLAIR. CT brain and CT perfusion: presence of hypo-perfused salvageable regions of brain detected on automated perfusion imaging using RAPID software. perfusion (lesion–ischemic core) mismatch is defined as a ratio of greater than 1.2 , an absolute difference greater than 10 ml, and an infarct core lesion volume of less than or equal to 70 ml. CT brain and CT cerebral angiography: ASPECTS =6 and good collateral status grading score in CT angiography

Exclusion criteria

Exclusion criteria: NIHSS >25. Poor collateral grading score on digital subtraction angiography. Stroke involving more than 1/3 of the middle cerebral artery territory on MRI brain.

To evaluate the safety and efficacy of combined intravenous thrombolytic therapy and mechanical thrombectomy versus mechanical thrombectomy alone in patients with acute ischemic stroke due to large vessel occlusion presented beyond the standard 4.5-hour’ time window.

MeasureTime frame
to determine the effect of IV thrombolytic therapy on mechanical thrombectomy procedure and achieving first-pass effect
