Inclusion criteria

Inclusion criteria: 1. 1. Type 2 diabetes mellitus patients confirmed by any of the following criteria at diagnosis: (1) symptoms of diabetes (polyuria, polydipsia, polyphagia, weakness, blurred vision) and random plasma glucose concentration = 200 mg/dl (11.1 mmol/l), (2) fasting plasma glucose = 126 mg/dl (7.0 mmol/l), (3) 2-hours plasma glucose = 200 mg/dl (11.1 mmol/l) during an oral glucose tolerance test, 18 years and above who attended GOPC, JUTH, with a diagnosis made at least three months prior to the conduct of the study 2. Patients who had been on at least one oral glucose lowering medication for at least three months in the clinic to ensure familiarity with prescribed medications 3. Patients who consented to the study and signed the consent form

Exclusion criteria

Exclusion criteria: 1. Type 2 diabetic patients confirmed by any of the following criteria at diagnosis: (1) symptoms of diabetes (polyuria, polydipsia, polyphagia, weakness, blurred vision) and random plasma glucose concentration = 200 mg/dl (11.1 mmol/l), (2) fasting plasma glucose = 126 mg/dl (7.0 mmol/l), (3) 2-hours plasma glucose = 200 mg/dl (11.1 mmol/l) during an oral glucose tolerance test, with other co-morbidities (heart failure, cerebrovascular accident, chronic kidney disease, cognitive impairment) 2. Patients too ill to withstand the intervention 3. Patients residing outside Jos to reduce attrition 4. A second member of a household in which a member had been included in the study
