Inclusion criteria

Inclusion criteria: 1. Older adults (both male and female), aged 65 years and above, who attend the Geriatric Clinic at AKTH. 2. Participants must be physically ambulant (with or without walking aid). 3. Participants must be able to communicate in English or Hausa. 4. Older patients who are willing to provide informed consent.

Exclusion criteria

Exclusion criteria: 1. Older Patients with cognitive impairment may not be able to understand or comprehend well. This will be assessed through a mini mental state examination conducted for all participants at baseline. 2. Severely ill patients whose conditions would not allow them to be interviewed on review dates. 3. Older patients with behavioural disorders (such as schizophrenia) may interfere with counselling. 4. Older patients with communication impairments, such as those who are deaf or non-verbal, will be excluded due to challenges in administering the questionnaire and delivering the intervention effectively. 5. Older patients unable to adhere to the 4-weekly follow-up schedule will be excluded to minimize the risk of attrition.
