Inclusion criteria
Inclusion criteria: a. For the Older Persons Discharged from UBTH 1. Age 60 years and above (older patients). 2. The presence of at least two chronic illnesses requiring regular physician follow-up. 3. Being on at least 4 routine medications. 4. Informed consent. 5. Possession of, or access to, an active smartphone. 6. Consent from the managing physician. 7. Ability of the patient to communicate with the researcher. b. For the Caregiver 1. Informed consent. 2. Possession of/access to an active smartphone by patient and/or caregiver. 3. Ability of the caregiver to communicate with the researcher.
Exclusion criteria
