Inclusion criteria
Inclusion criteria: Patients aged over 65 years who are currently prescribed more than five chronic medications Participants must be receiving care in either outpatient or inpatient services at one of the participating centers. Participants must be able to provide informed consent directly or via a legally authorized representative.
Exclusion criteria
Exclusion criteria: Patients who are terminally ill or receiving palliative care Patients with severe cognitive impairment (e.g., advanced dementia) indicated by a score below 18 Individuals currently enrolled in another interventional clinical trial An individual with a known hypersensitivity or aversion to digital or electronic health interventions.
