Exclusion criteria: • Large bowel obstruction • Incarcerated abdominal wall hernia • Postoperative ileus (within 6 weeks) • Active abdominal oncological process (primary tumour, metastasis, peritoneal carcinomatosis) • Patients without CT scan or with a CT scan without contrast • Patients post radiotherapy • Intraabdominal infection • Acute mesenteric ischemia
Design outcomes
Primary
| Measure | Time frame |
|---|---|
| Validation of the Wassmer 8-item score for detecting bowel ischemia requiring surgical intervention in patients with acute adhesive small bowel obstruction, with treatment decision (operative vs. conservative) guided by the score. Each patient evaluated through eight item score which includes the following (age 70 years and older ,first attack of small bowel obstruction, absence of bowel movement for 3 days and more ,abdominal guard or rigidity,CT transition zone ,CT lack of bowel wall enhancement ,CT amount of IPFF 500 and more ml patients scored 4 and more explored immediately and others scored less than 4 underwent conservative management | — |
Secondary
| Measure | Time frame |
|---|---|
| Postoperative complications for operated cases which includes patients operated upon score from start and patients didn’t respond to conservative magement by clavien dindo score ,other predictors for bowel ischemia in acute adhesive bowel obstruction other than the items in score applied in our study | — |
