Inclusion criteria

Inclusion criteria: - Adults (=18 years) with decompensated cirrhosis (confirmed by histology or clinical/imaging/biochemical evidence). - Presence of ascites (detected on abdominal ultrasonography). - Hospitalized =24 hours (to capture early AKI biomarkers). - Willingness to provide informed consent (or consent from legally authorized representatives for encephalopathy patients).

Exclusion criteria

Exclusion criteria: - ESRD (eGFR <15 mL/min/1.73 m² or dialysis). - Hepatocellular carcinoma (multifocal). - Recent GI bleeding (<2 weeks). - Transplant recipients.

Design outcomes

Primary

MeasureTime frame
Diagnostic accuracy of serum NGAL for early AKI detection in decompensated cirrhosis (measured by AUC, sensitivity, specificity at cutoff >150 ng/mL).

Secondary

MeasureTime frame
Discriminatory power of RRI (>0.74) for hepatorenal syndrome (HRS) identification (AUC, specificity). 30-day mortality prediction by NGAL (>150 ng/mL) and RRI (>0.74) (adjusted HR via Cox regression). Cost-effectiveness of NGAL-guided ICU triage (reduction in unnecessary transfers by 28%).
