Inclusion criteria
Inclusion criteria: Patients suffering from a posterior circumscribed ulcer accompanied by a sentinel skin tag, indurated edges, and exposure of horizontal fibers of the internal anal sphincter, along with symptoms of post-defecatory or nocturnal pain, bleeding, or both, persisting for over two months.
Exclusion criteria
Exclusion criteria: 1- acute anal fissure or recurrent anal fissures 2- Fecal incontinence 3- Associated anal pathologies (e.g., hemorrhoids, anal fistula, anorectal abscess, or rectal cancer) 4- Atypical fissures caused by inflammatory bowel disease 5- Prior anal or rectal surgery 6- Refusal to participate in the study 7- Conditions impairing wound healing (e.g., uncontrolled diabetes with HA1c = 7%, use of immunosuppressive drugs) 8- Patients classified as American Society of Anesthesiologists (ASA) Class > II
