Inclusion criteria
Inclusion criteria: 1. Diagnosis of HTN and elevated SBP measure (=140 mm Hg) on their most recent clinic visit (within the previous 6 months) without a diabetes diagnosis between the ages of 18 -70 years 2. Diagnosis of HTN and elevated SBP measure (=140 mm Hg) on their most recent clinic visit (within the previous 6 months) without a diabetes diagnosis 3. Receives primary care at one of the participating sites 4. Willing and able to provide informed consent in English, Twi, Dagbani or Yoruba 5. Owns a smartphone (Android or iOS)
Exclusion criteria
Exclusion criteria: 1.Age 70 years 2. Severely elevated BP (=180/110mmHg) 3. Diagnosis of stroke, coronary artery disease, or kidney disease 4. Diagnosis of end-stage renal disease (ESRD) treated with dialysis 5. Serious medical condition which either limits life expectancy or requires active management (e.g., cancer) 6. Cognitive impairment or other conditions preventing study participation 7 .Pregnant or currently nursing a child 8. Unwilling to provide informed consent
