Inclusion criteria

Inclusion criteria: ¿ Patients scheduled for major lower limb orthopedic procedures exceeding 4 hours as bilateral both lower limbs procedures or two procedures in one lower limb with possible bone grafting. ¿ ASA I-III. ¿ Both sexes.

Exclusion criteria

Exclusion criteria: ¿ Patient refusal. ¿ Hypersensitivity to used anesthetics, ¿ General contraindications to spinal anesthesia, ¿ Cardiac, hepatic, renal or respiratory failure. ¿ Difficult communication.

¿ The required doses of intraoperative iv midazolam, fentanyl and propofol. ;¿ The rate of conversion to general anesthesia.;Intraoperative hemodynamics (MBP, HR) ;postoperative hemodynamics (MBP, HR) ;Maximum and minimum MBP and HR ;Required doses of atropine and ephedrine ;Incidence of using oxygen face mask (SaO2 less than 90%).;estimated blood loss ;The number of packed RBCs transfusion units;The volume of fluids administration ;Incidence of shivering, itching, nausea & vomiting;Incidence of sedation, shivering, itching, nausea & vomiting;The total morphine requirements ;Visual analogue scale (VAS);Duration of hospital stay;Patient satisfaction in a score (0 - 10)
