Inclusion criteria

Inclusion criteria: adult males American Society of Anesthesiologists (ASA) physical status class I¿II patients scheduled for testicular biopsy for sperm extraction

Exclusion criteria

Exclusion criteria: refusal of regional anesthesia a known allergy to (bupivacaine, dexmedetomidine, non steroidal anti inflammatory drugs or morphine) bleeding disorders infection in the inguinoscrotal region , a history of drug or alcohol abuse and patients suffering from chronic pain patients regularly receiving analgesics.

The primary end point was the rate of success of the block. The block was considered unsuccessful if any additional local anesthetic, analgesic or sedative were used, and or VAS score was <3 during operation, and or conversion to general anesthesia	—
