Inclusion criteria

Inclusion criteria: The individual is resident in RotterdamThe individual takes part in the MoR programme and has passed the active phase (so that the individual received care for a substantial time).The individual has partaken in an interview or focus group discussion as part of the MoR programme evaluation.The individual is highly vulnerable according to the MoR intake The individual needs to be sufficiently proficient in Dutch OR a social care provider working within the MoR program. 

Exclusion criteria

Exclusion criteria: The individual is unwilling or unable to give informed consent to the studyThe care is completely taken over by a different organization
