Inclusion criteria
Inclusion criteria: New outpatient clinic patient visit scheduled at participating departmentsDutch or English-speakingAge =18 years (or parental/legal guardian consent for minors, if including pediatrics) Able and willing to provide informed consent
Exclusion criteria
Exclusion criteria: Cognitive or language impairments preventing meaningful use of Ditto app or survey completionSevere visual, auditory or motor impairments precluding interaction with app or survey completion

| Response variablesIPQ-K questionnaire – A validated instrument used to assess patients’ cognitive and emotional representations of their illness, available in Dutch and English. Possible score range 7–70, with higher scores indicating better understanding and more coherent illness perception. Note: Question 7 (“In welke mate vindt u dat u uw ziekte begrijpt?”) will form the primary outcome, while the full questionnaire score will serve as a secondary outcome. Patient Activation Measure (PAM-13) – A validated 13-item questionnaire used to assess patients’ knowledge, skills, and confidence in managing their health. Possible score range 0–100, with higher scores indicating greater activation and engagement in care. Usability measures (intervention arm only) - A brief, focused set of questions assessing the perceived usefulness, clarity, and ease of use of the Ditto app (Appendix 1). Items were selected to minimize overlap with PAM-13 and IPQ-K. Responses are measured on a 0–100 slider scale (0 = strongly disagree, 100 = strongly agree), generating a total usability score ranging from 0–500.
