Eligibility

Age
2 Years to 17 Years

Inclusion criteria

Inclusion criteria: Children aged 2-18 years old, admitted to the paediatric ward with acute reversible bronchoconstriction exacerbations due to acute asthma- and viral wheeze exacerbations, reversible bronchial hyper reactivity or reversible bronchoconstriction with- or without additional lower airway infection. This includes children referred from paediatric intensive care which have been treated for the very severe phase of the bronchoconstriction attack and are now further treated on the paediatric ward.  Informed consent obtained.

Exclusion criteria

Exclusion criteria: Need for other care requiring continuous physician supervision, making treatment via the nurse-led pathway impossibleNear-fatal asthma or viral wheeze, previously treated with resuscitation, mechanical ventilation, or ECMOChronic or congenital pulmonary disorders other than asthma or recurrent viral wheezeNeurological, muscular, or other severe disorders that make treatment via the nurse-led pathway impossible (e.g., psychomotor retardation, Down syndrome, hemodynamically significant cardiac disease)Physician judgment that the patient cannot be treated via the nurse-led pathway

Design outcomes

Primary

MeasureTime frame
Time from the moment the treating physician has concluded that the initiated treatment in the emergency ward is effective and considers the patient sufficiently stable to allow continuation of care on the general paediatric wardtoFor three hours no bronchodilators have been administered andIn the next 6 hours, no interval between bronchodilators was less than 3 hours, or the patient has been discharged

Secondary

MeasureTime frame
The quality of treatment: Effectiveness of the treatmentSafety of the treatmentBurden on patient and parents Empowerment of patient and parentsCost-effectiveness from patient/family, healthcare system, and societal perspectives.
