Inclusion criteria

Inclusion criteria: Family caregivers: age 18 years or older who received guidance by the PPCT Sophia in the last three yearsHealthcare professionals: at least two years of experience in delivering home-care to patients and families with life-limiting and life-threatening diseases. Professionals that are experts in logistics, technologic and legal issues who work in the Erasmus MC.All participants should be able to understand and speak the Dutch language to participate in focus group discussions. Participants who do not speak Dutch, but who do speak English or another language for which an interpreter is available, will be asked to participate in an interview. All participants should be able to participate in interviews and/or focus group discussions.

Exclusion criteria

Exclusion criteria: Healthcare professionals: limited experience (< 2 years) in delivering home-care to PPC patients.Family caregivers of patients who were not guided by the PCCT of Erasmus MC – Sophia in the past 3 years.Family caregivers of patients that deceased during the last six months before start of the study.

The metromap ‘AS-IS’ and metromap ‘TO-BE’, drafted during the co-creation sessions and finalized based on the content analysis.The preferences of families regarding the use of telehealth solutions for palliative care in the home setting.Facilitators and barriers for the use of telehealth solutions in the home setting.

MeasureTime frame
A stakeholder map identifying health care professionals who participate in pediatric palliative home-care will be created by the PPCT of the Erasmus MC Sophia Children’s Hospital who has extensive knowledge of the pediatric palliative home-care in the Southwest region of the Netherlands.An overview of legal, logistical and technical possibilities and limitations regarding the telehealth solutions added to the TO-BE timeline will be reported after conversations with the stakeholders in the hospital.
