Inclusion criteria
Inclusion criteria: Patients at Rijnstate Hospital scheduled to initiate kidney replacement therapy (defined as the start of the dialysis preparation pathway as determined by the treating nephrologist, with an estimated clearance <20 mL/min), including both hemodialysis and peritoneal dialysis.Age = 65 years.
Exclusion criteria
Exclusion criteria: Patients for whom a conservative (non–kidney replacement therapy) treatment approach has been established.Patients requiring short-term dialysis for acute kidney injury (e.g., temporary bridging on the intensive care unit).Inability to provide informed consent for study participation.

| Primary Endpoints: Identification of predictors associated with a more rapid decline in frailty (assessed using the Clinical Frailty Scale) and cognitive function (assessed using the MoCA score). To this end, the baseline CFS and MoCA scores will be compared with the last available scores prior to study discontinuation (e.g., death, cessation of dialysis, loss to follow-up, etc.). | — |
