Eligibility

Age
18 Years to 99 Years

Inclusion criteria

Inclusion criteria: Adult (male/female = 50 years of age) patientsAdmission to the ICU = 7 daysOrgan failure defined as the need for invasive ventilation or the necessity of vasopressor therapy.Either inflammation/infection are parameters persistently elevated. Defined as a CRP >75 mmol/L and no persistent decrease of >50% in the past three days. OR the patient receives tacrolimus, cellcept, azathioprine (AZA), anti-thymocyte globulin (ATG), basiliximab, tocilizumab or high dose/continuous corticosteroidsPET transport deemed safe by attending clinician (staff intensivist)Informed consent signed by patient or legal representative

Exclusion criteria

Exclusion criteria: Women who are pregnant or breastfeeding.Known allergies to iodine contrast.Inability to attain informed consent.Preparatory failure defined as serum glucose >11 mmol/L before PET-scan.Too unstable for transport as judged by the treating staff intensivist.Patients suffering from isolated neurological pathology requiring ICU admission.Patients suffering from acute pancreatitis requiring ICU admission.Patients receiving extra-corporeal membrane oxygenation (ECMO).

Design outcomes

Primary

MeasureTime frame
To evaluate incidence of [18F]FDG PET/CT findings compared to contrast-enhanced CT in patients with persistent critical illness.

Secondary

MeasureTime frame
Secondary Objective(s): To evaluate the incidence of therapy change following [18F]FDG PET/CT.To explore the relationship between [18F]FDG PET/CT findings and circulating biomarkers.Tertiary Objective(s):To estimate the effect of imaging on ICU LOS, ICU- and hospital mortality and the length of required organ support using a retrospective matched cohort from 2023-2024.
