Age
18 Years to 99 Years

Inclusion criteria

Inclusion criteria: Admitted to the adult ICU for more than 24 hours;Admitted to the adult ICU for less than 24 hours but still received a pharmacist’s review and/or intervention.

Exclusion criteria

Exclusion criteria: Patients who have filed an objection to the use of their data from the Electronic Patient Dossier for medical research purposes.

MeasureTime frame
To determine the clinical and financial impact of two different levels of Dutch ICU pharmacy service, made by the pharmacist (1) during patient rounds or multidisciplinary team (MDT) meetings / in the ICU, (2) following electronic medication surveillance, (3) after consultation and (4) during laboratory therapeutic drug monitoring (TDM).
