Inclusion criteria

Inclusion criteria: Age 18 years or more;Referred to the cardiology clinic of the participating center for HF-like symptoms - as stated in the ESC 2016 Guidelines;H2FpEF score of 3 or more

Exclusion criteria

Exclusion criteria: Known with a history of HF (despite LVEF);Known with a history of severe valvular or congenital heart disease;

Primary

MeasureTime frame
The primary endpoint is the number of patients diagnosed with HFpEF at 6 months and 12 months follow-up, compared to the initial H2FPEF score (5 or less versus 6 or more). 

Secondary

MeasureTime frame
Secondary endpoints are: availability of echocardiographic parameters in clinical reports; Characterization and overlap/discrepancy among H2FPEF, HFA-PEFF, and HFpEF-ABA likelihood scores; Guideline-directed optimal medical therapy use at 6 and 12 months, stratified by diagnosis and likelihood scores; HF events and mortality, stratified by diagnosis and likelihood scores; (Incremental) Diagnostic and/or prognostic value of clinical parameters, developing or improving existing models.
