18 Years to 64 Years

Inclusion criteria

Inclusion criteria: Adult psychiatric patients (18+ years), admitted to a participating forensic psychiatric institute. Presence of a psychotic disorder according to the Diagnostic and Statistical Manual of Mental Disorders 5 criteria (DSM-5; American Psychiatric Association, 2013). Sufficient understanding of the Dutch language. Capable of making an informed decision regarding participation in this study.

Exclusion criteria

Exclusion criteria: Patients with neurodegenerative diseases such as Alzheimer, Parkinson, Korsakov, etc), due to the non-reversible cognitive deficits associated with these neurodegenerative disorders. Patients who are not able to leave the unit to visit the music therapy room (For example, when the severity of the acute risk to themselves or others is so great that patients cannot leave the ward to attend music therapy. Patients who are unable to visit the music therapy room due to immobility are also excluded from participation in this study) Patients who undergo elective compulsory therapy, because of its negative effects on cognitive functions in the first month after the start of treatment (Landry et al., 2021; Yoldi-Negrete et al., 2022) . Patients who are about to be leave the institute within half a year after baseline assessment.

To compare the effectiveness of Musical Attention Control Training (MACT) with Supportive Music Therapy (SuMT) after twelve sessions in improving cognitive functions in patients with psychosis spectrum disorders. Hypotheses: MACT patients are expected to perform better on cognitive tasks after twelve sessions compared to SuMT patients. Improvements will be measured at mid-training (six weeks) and end-training (twelve weeks), with MACT patients expected to score higher than SuMT patients. The effect of MACT will be reassessed after a six-week follow-up, and it is expected to be maintained.

1 Compare the effectiveness of MACT and SuMT over twelve weeks for reducing clinical symptoms and improving functional outcomes in patients with psychosis spectrum disorders. Hypotheses: After twelve sessions, MACT patients will show greater reductions in positive and negative symptoms than SuMT patients. MACT patients will show greater improvement in functional outcomes. 2 Assess whether MACT influences treatment responsiveness and adherence by monitoring incidents, additional medication use, adherence to other treatments, and dropout rates. Hypothesis: MACT patients will show better treatment responsiveness and adherence than SuMT patients.
