Interventions

NanoNeedle mini-arthroscopy of the MCP-hand joints is not yet standard&nbsp

treatment of care, but is currently used for arthroscopy of other joints. So&nbsp

far, NanoNeedle mini-arthroscopy has been performed in/is being used for joint&nbsp

pathology of the wrist joints (Dr. N. Schep, handsurgeon) and in ankle/foot,&nbsp

schoulder, knee and wrist joints (Prof. G. Kerkhoffs, orthopaedic surgeon,&nbsp

Amsterdam UMC). Access of the NanoNeedle to the MCP joint space allows inspection and surgical&nbsp

removal of inflamed synovial tissue using a mini shave. Mini-arthroscopy is&nbsp

performed as a day case by our hand- and wrist surgeon with high-level&nbsp

experience using the 1.9mm semi-flexible NanoNeedle (Arthrex) and accessory&nbsp

instruments. All NanoNeedle arthroscopies are performed in the Maasstad&nbsp

hospital in Rotterdam and Spijkenisse Medisch Centrum. Postoperatively, a&nbsp

pressure dressing will be applied for 48 hours. Patients can mobilize the hand&nbsp

immediately. The procedure will take approximately 60 min.

18 Years to 64 Years

Inclusion criteria

Inclusion criteria: Inclusion criteria patients with inflammatory arthritis * Male and female patients with age >= 18 years  * Patients that are diagnosed with RA (according to the revised 2010 ACR/EULAR  Rheumatoid Arthritis Classification Criteria), PsA (according to the CASPAR  criteria) or SPA by the rheumatologist.  * Nonresponding mono- or oligoarthritis of MCP-hand joints, defined clinically or by imaging modality (ultrasound or MRI)  * Indication for therapeutic synovectomy by NanoNeedle mini-arthroscopy because  of arthritis of the MCP-hand joints, as established by rheumatologist and  handsurgeon. Inclusion criteria patients with non-inflammatory indication for NanoNeedle  arthroscopy * Male and female patients with age >= 18 years  * Patients not known with inflammatory arthritis * Indication for therapeutic synovectomy by NanoNeedle mini-arthroscopy of the  MCP-hand joints, as established by rheumatologist, hand or orthopedic surgeon.

Exclusion criteria

Exclusion criteria: * No consent from the patient to fill out the VAS painscore and complete two questionnaires; under 10-15 min to complete.  * Insufficient proficiency at Dutch language/ insufficient skills to understand  Dutch language.

Measure	Time frame
The primary objective is the patient VAS-painscore (before and after mini-arthroscopy (NanoNeedle) of MCP-hand joints in patients with difficult to treat mono- or oligoarthritis, related to RA, PsA and SPA. The patient VAS-painscore will be assessed at the first encouter (when signing the informed consent) and at postsurgery wound control (10-14 days af er arthroscopy).	—
