Inclusion criteria

Inclusion criteria: - Children of 3 - 18 years of age with a positive open or double-blind placebo-controlled peanut or nut challenge =0.35 kU/l or skin prick test > 3 mm to peanut or nut, determined

Exclusion criteria

Exclusion criteria: - Only mild symptoms in the oral cavity to peanut or nut due to pollen food syndrome; - A negative peanut or nut challenge; - Children who are not potty trained (house trained); - Gastro-intestinal diseases (e.g. Morbus Crohn, celiac disease, Colitis Ulcerosa), lactose-intolerance, severe cow's milk allergy genetic syndromes, infectious/immunology diseases other than atopy, or diabetes; - Use of laxantia, such as lactulose; - Not being able to read or write Dutch

Design outcomes

Primary

MeasureTime frame
- Changes in small intestine permeability after 4 months of diet intervention as expressed by the Raffinose/Maltitol ratio (R/M ratio) in urine.

Secondary

MeasureTime frame
Clinical assessment: · The relationship between gut permeability (R/M ratio) and the severity of coexisting allergic symptoms; · Differences in severity of atopic dermatitis measured (SCORAD-score) . Differences in gut microbiota composition between baseline and after 4 months of dietary intervention; · The relationship between gut permeability (R/M ratio) and threshold level to peanut or nuts established during the oral food challenge (cumulative mg peanut or nut protein). Patient reported outcome: · Differences in atopic dermatitis (POEM), asthma (Caratkids), upper respiratory tract infections (days, doctor's visitis, antibiotics, days off from school by child or work by parents), gastro-intestinal symptoms (Birmingham IBS questionnaire), and use of medication (Medication Registration Form, MRF); · Differences in food allergy Quality of Life measured by disease-specific age-related quality of life questionnaires (HRQLQ-PF, HRQLQ-CF); · Self-assessed feasibility of adherence to the Healthful Diet and dietary compliance by likert scale after 2,5 and 4 months of dietary intervention. Nutritional intake: · The association between nutritional intake (nutrients and foods), assessed by three online 24-hour food recalls and gut permeability (R/M ratio) . Calculated compliance by the researcher by the number of first and second choice foods in 3-days food diaries after 4 months of dietary intervention.
