Inclusion criteria
Inclusion criteria: inclusion criteria: - > 18 years - legally capable & - diagnosis of psychiatric/social/chorniscal physical illness (2 out of 3) AND Phase 1: more than average acute health care use (2x/year) Phase 2: average acute health care use (
Exclusion criteria
Exclusion criteria: exclusion criteria: - no informed consent of the patient (asked by the own GP) - terminally ill (determined by own GP) - dementia in a stage where communication is not trustworthy (determined by own GP) - legally incapable (determined by the GP), After the exclusion phase the researchers get the list of the included study subjects.
