Inclusion criteria

Inclusion criteria: - Elderly, 65 years of age and older - Community-dwelling - Referral from the general practitioner due to chronic condition - FAC 3 or more

Exclusion criteria

Exclusion criteria: If an older adult is insufficiently instructible after assessment by the physical therapist, he or she can not participate in this study.

MeasureTime frame
Fear of falling will be mapped with the Falls efficacy scale international (FES-I). Physical activity will be measured by an accelerometer. The Fitbit Classic will be used in this study. Seen the limitations of the Fitbit Classic (it is not suitable in water), physical activity will also be measured through the Lasa Physical Activity Questionnaire, in short LAPAQ. The physical capacity will be measured through the Short Physical Performance Battery, in short SPPB. Balance will be measured through the Timed up and go test, in short TUGT. Muscle strength of the lower extremities will be measured through the Timed Chair Stand Test, in short TCST. Endurance will be measured through the 2-minute step test, in short 2MST. Flexibility will be measured trough the Chair Sit and Reach Test, in short SRT.
