Inclusion criteria

Inclusion criteria: • Age over 18 years; • Informed Consent; •

Exclusion criteria

Exclusion criteria: • Psychopathology or severe cognitive impairment • Combined organ transplantation • Physical or clinical limitations that prevent participation to the intervention program. • Negative results of medical screening by the nephrologist or cardiologist.* • Pregnancy
