Eligibility

Age
18 Years to 99 Years

Inclusion criteria

Inclusion criteria: Patients will be eligible for inclusion if they are 18 year or older, and have a clinical diagnosis of TIA or ischemic stroke

Exclusion criteria

Exclusion criteria: Patients will be excluded if they are discharged to a nursing home, are non-Dutch speaking or notably aphasic.

Design outcomes

Primary

MeasureTime frame
Primary outcome measures will be changes in physical activity, dietary behavior, and smoking cessation at 3 months

Secondary

MeasureTime frame
Secondary outcome measures will include body mass index and waist circumference at 3 months.
