Inclusion criteria
Inclusion criteria: o Patients with high suspicion for a primary or secondary brain malignancy on prior imaging o Patients have not received any invasive intervention yet o Informed consent o Adult patients >= 18 year. o MDRD > 60

Exclusion criteria: o Patients with known contra-indications for MRI or contrast-enhanced CT o Patients with previous therapy for brain tumors o Pregnancy

| Visual inspection of the perfusion maps is used to assess image quality. Quality of the images will be assessed by two experienced neuroradiologists. From each patient parametric maps of CBV, CBF, MTT and TTP will be generated from CTP and MRP images. Evaluation of image quality will be based on the visualization of various anatomic structures and on some general image quality criteria described below. The anatomic structures used for evaluation can be sufficiently visualized on standard MR sequences, which are part of the acquisition sequence in each patient. This information can be used as an internal reference for both, CT and MR perfusion datasets. Evaluation of image quality on parametric maps is performed using a rating scale for each of the following anatomic features: • white-gray matter differentiation • differentiation of basal ganglia • blur, unsharpness or blooming of vessels • differentiation of the ventricles and the following general features: • Overall image quality • image distortion and artifact • spatial resolution • image noise. The precise choice of scale will be determined once the first datasets have been acquired to make sure there is enough discriminating power. | — |

| All images registered (corrected for differences in size, patient motion, etc) so that one image can be used to draw Regions of interest*s (ROI) in homogenous areas: • Gray matter • White matter • CSF • Plexus choroideus • Thalamus • Nucleus caudatus • capsula interna • arteries • sinus sagittalis superior Tumour regions: • total tumor • tumor necrosis • tumor hot spot • peritumoral edema ROI in tumour regions will be manually identified in each patient. From these ROI*s at least the following perfusion parameters will be assessed from CTP and MRP: CBV, CBF, MTT, TTP. ROI placement and quantification of the perfusion parameters will be assessed by the principal investigator in consensus with an experienced neuroradiologist. In each patient perfusion parameters of CTP and MRP will be measured and correlated. | — |
