18 Years to 99 Years

Inclusion criteria

Inclusion criteria: patients with moderate to very severe COPD (GOLDII/III/IV) patients with very severe chronic heart failure (NYHA III/IV) patients with very severe renal failure (requiring dialysis)

Exclusion criteria

Exclusion criteria: The patients is not clinically stable for at least 4 weeks preceding enrolment Pharmacological therapy is not optimal (according to the current available guidelines) and stable for at least 2 months preceding enrolment. Patients in a nursing home.
