Inclusion criteria

Inclusion criteria: CVA Discharched from Neurology ward

Exclusion criteria

Exclusion criteria: Patients with impaired level of consciousness, with severe problems in language comprehension, multiple cognitive impairments reflecting dementia syndrome, as well as patients who were in peril of death are excluded.
