Inclusion criteria
Inclusion criteria: The patient: - stroke is the main cause of his dependency and has occurred longer than 3 months ago. - lives in a nursing home in a 'somatic unit' (a unit for residents who have mainly physical handicaps);The informal caregiver: - is the person to be contacted or representative of the patient. - is closely in touch with the care for the patient.;The professional: - is responsible for the daily care of the patient
Exclusion criteria
Exclusion criteria: The patient: - has severe communication deficits - can not be loaded due to severe depression or bad physical condition - is not able to give informed consent
