Inclusion criteria

Inclusion criteria: patients; 1) visiting the outpatient clinic of the department of Cardiology (a cardiologist or specialized nurse), 2) age >= 18, 3) willing to give informed consent prior to completion of the questionnaires.

Exclusion criteria

Exclusion criteria: patients not being able to speak or read the Dutch language
