het onderzoek wordt uitgevoerd bij mensen die zijn ontslagen van de afdelingen cardiologie, interne geneeskunde of longgeneeskunde. Drug related problems after discharge from hospital

Interventions

A systematic medication review by community pharmacists. Pharmacy technicians will counsel patients at home at baseline and at 1, 3, 6, 9 and 12 months. Control patients receive usual care.

Eligibility

Age

18 Years to 99 Years

Inclusion criteria

Inclusion criteria: Age: over 60 years Drugs: over 5 chronic prescription only drugs Discharged from hospital from the departments of cardiovascular disease, lung disease and internal medicine

Exclusion criteria

Exclusion criteria: - Patients discharged to a nursing home - A poor mental status - Terminal ill patients - Patients not able to communicate in Dutch
